Client Consent

I, Alexandra Jacobs (Ali/Anahit), a facilitator, coach, and Licensed Massage Therapist, operate under the Colorado Natural Health Consumer Protection Act and DORA (Colorado Department of Regulatory Agencies). My practice includes complementary and alternative therapies, such as somatic coaching, energy medicine, therapeutic massage, and embodiment practices. I am not a licensed counselor or medical professional, and the services provided are for personal growth and healing.

As the client, you are the decision-maker in your healthcare. I provide information to assist you in making informed choices about your wellness. By engaging in sessions with Empowered Living LLC, you acknowledge and accept the scope of services offered, understanding that they do not substitute for medical or psychological treatment.

Privacy Policy

Your privacy is of the utmost importance. All personal information shared during sessions is confidential and will not be shared with third parties without your consent unless required by law. Records will be kept securely and only accessed by staff when necessary for treatment purposes.

Scope of Services

• The services provided by Empowered Living are designed for personal growth, healing, and transformation. These sessions are not a substitute for medical or psychological treatment.

Client Responsibility

• The success of our work depends on your active participation. You are responsible for your physical, emotional, and mental well-being throughout the process.

Confidentiality

• All personal information is kept confidential. Any disclosure required by law (such as harm to yourself or others) will be made accordingly.

 

Liability

• Empowered Living is not liable for any outcomes resulting from our services. For medical or psychological issues, please consult a licensed professional.

 

Payment and Cancellation Policy

• Cancellation and Rescheduling: Sessions must be canceled or rescheduled at least 24 hours in advance. Late cancellations will be charged 50% of the session fee. No-shows will be charged the full session price.

• Coaching Package Terms: Commitment is key to achieving desired outcomes. Clients are encouraged to remain focused and engaged. If the alignment no longer feels right, you may cancel at any time, with prorated refunds for remaining sessions.

 

Payment Policy

Full payment for coaching packages is required before the first session. If on a payment plan, the first payment is due before the first session, with subsequent payments due at agreed intervals.
